All Students who will be participating in Villa Maria Academy Sports must have a yearly physical done. This will be sufficient for all sports played that year. The form must be dated by at least January of the current year, but having it closer to the time your daughter will be playing her sport is the best.

The Physicals Form must be up-to-date and on file in the Nurse’s Office prior to the beginning of tryouts for that sport. A student will not be able to practice or play in a game if their physical form is not on file in the Nurse’s Office.
