Student Medical Forms

All New Students (PreK, Kindergarten, and transfer students) must have:

• A copy of Updated Immunizations

• Physical Exam
• Dental Exam

Prior to admission to school these forms must be on file. Students will not be admitted to school without these forms on file with the nurse. This is a PA State Requirement for admission to school.

Other PA State Medical Requirements include:

• Physicals for all 6^th Graders (due 1^st day of starting 6^th grade)

• Dental exams for all Kindergarten, 3rd,  and 7th Graders (also due 1^st day)

Make your daughter’s doctor and dentist appointments early in the summer and do not wait till school starts. All physical and dental forms returned to school by the first week of school.
